Co-Parenting Handover Forms

Professional templates for smooth handovers. Standard form, weekly schedule, emergency contact card.

£7

These forms are designed for co-parents who need clear, factual, professional documentation for handovers. Whether you're using them voluntarily or they've been recommended by a mediator or solicitor, these templates keep things consistent and protect everyone — especially the children.

How to use: Print the templates you need or save this page as a PDF. Fill them in before each handover. Keep copies. If you're in a high-conflict co-parenting situation, these records can be invaluable evidence of your cooperation and consistency.

Standard Handover Form

Use this at every handover. Takes 2 minutes to fill in. Creates a consistent record.

Why signatures matter: In high-conflict situations, a signed handover form is evidence that both parents were present, that information was shared, and that the children were handed over in good condition. If a dispute arises later ("you never told me about the medication"), you have written proof.

Important: Both parents with parental responsibility have the right to access their child's medical records and make decisions about medical treatment. In an emergency, hospital staff will treat without consent. For non-emergency treatment, consent from one person with parental responsibility is usually sufficient, but it's best practice to keep both parents informed.
